Clinical Evidence on CAD/CAM-Customized Meshes for Guided Bone Regeneration in Oral Implantology: A Review of Clinical Studies

Guided bone regeneration (GBR) is widely used to reconstruct alveolar ridge deficiencies prior to implant placement; however, maintaining a stable regenerative space remains challenging, particularly in vertical and combined defects. Recent advances in computer-aided design and computer-aided manufacturing (CAD/CAM) have enabled the development of patient-specific customized meshes to enhance graft stability and clinical outcomes. This narrative review aimed to synthesize clinical evidence regarding CAD/CAM-customized meshes used in GBR procedures. Human clinical studies, including randomized trials, prospective and retrospective studies, case series, and case reports, were analyzed. Thirteen clinical studies were included. Most investigations evaluated customized titanium meshes, whereas polymer-based alternatives, such as polyetheretherketone (PEEK), were less frequently reported. Overall, favorable clinical and radiographical outcomes were observed, with adequate vertical and horizontal bone gain enabling subsequent implant placement in complex defects. Mesh exposure and wound dehiscence were the most common complications; however, bone regeneration was often successfully achieved with appropriate management. Despite limitations in heterogeneous study designs and outcome reporting, CAD/CAM-customized meshes appear to be a reliable approach for guided bone regeneration. Further well-designed prospective clinical trials are required.
