Investigation of antibiotic resistance of Klebsiella pneumoniae isolates from samples obtained from Al-Zahra hospital in Isfahan and the frequency of wabG gene in the extensively drug resistant (XDR) isolates in ۲۰۲۳

Antibiotic resistance and virulence progress in Klebsiella pneumoniae is a global challenge. This study aimed the isolation of antibiotic-resistant strains of Klebsiella pneumoniae from the patients in different wards of Al-Zahra hospital in Isfahan and detection of XDR and multi-drug resistant (MDR) isolates and determination of the frequency of a virulence gene (wabG) in XDR isolates. Sampling was done from March to December ۲۰۲۳. Antibiotic resistant pattern was determined by Kirby–Bauer disk diffusion method. XDR and MDR strains were detected according to laboratory standards institute (CLSI) ۲۰۲۴. wabG gene in XDR isolates was detected by PCR. Among ۱۰۰ clinical strains of Klebsiella pneumoniae ۴ strains (۴%) were XDR and ۶۸ strains (۶۸%) were MDR. The frequency of antibiotic-resistant strains was higher in men than women. The most antibiotic-resistant strains were found in the age group upper than ۵۰ years and from urine samples in ICU and infectious wards. The most resistance was obtained to amoxicillin and ampicillin (۷۸%), cephazolin (۷۴%) and cefixime (۶۴%). wabG gene was found in ۵۰% of XDR strains. XDR frequency and urinary tract infection which was obtained in this study is of concern. The high prevalence of wabG gene in XDR isolates is important in the probable transition of this gene in nosocomial infections.
