Infertility is one of the most significant reproductive health challenges worldwide, imposing not only substantial medical burdens but also profound psychological consequences on affected women. Evidence indicates that women undergoing assisted reproductive technologies (ART), including in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI), experience significantly higher levels of anxiety, depression, chronic psychological stress, and reduced quality of life compared with the general population. Recent advances in Generative Artificial Intelligence (Generative AI) and Large Language Models (LLMs), such as ChatGPT, have introduced new opportunities for delivering accessible digital psychological support. These technologies offer conversational interactions, psychoeducational guidance, and preliminary emotional support, potentially addressing gaps in conventional mental health services for women undergoing infertility treatment. The present study aimed to systematically review and thematically synthesize the current evidence regarding the role, effectiveness, limitations, and ethical implications of LLM-based interventions in the psychological support of women receiving infertility treatment.This systematic review was conducted following the PRISMA ۲۰۲۰ guidelines. Electronic searches were performed in PubMed, Scopus, Web of Science, PsycINFO, and Google Scholar to identify studies published between ۲۰۱۹ and ۲۰۲۶. After removing duplicate records and completing title, abstract, and full-text screening, studies investigating the application of artificial intelligence in mental health and infertility care were included. Owing to the emerging nature of LLM applications in reproductive medicine, findings were synthesized using a combined narrative review and thematic synthesis approach.
