The Impact of Human Papillomavirus (HPV) Type on Clinical Outcomes in Patients with Laryngeal Cancer

Introduction Laryngeal cancer, a significant malignancy affecting the upper respiratory tract, has been increasingly associated with Human Papillomavirus (HPV) infection. Understanding the impact of different HPV types on the clinical outcomes of laryngeal cancer patients is crucial for optimizing treatment strategies and improving prognosis. Material and Method This study involved a sample of ۱۵۰ patients diagnosed with laryngeal cancer, comprising ۹۰ males and ۶۰ females, with a mean age of ۵۵.۴ years. The presence and type of HPV infection were determined through polymerase chain reaction (PCR) and subsequent sequencing. Patients were monitored over a period of five years to assess clinical outcomes, including tumor recurrence, metastasis, and overall survival rates. Results Among the ۱۵۰ patients, ۷۵ were found to be HPV-positive. The distribution of HPV types was as follows: ۴۰ patients with HPV-۱۶, ۲۰ with HPV-۱۸, and ۱۵ with other HPV types. The five-year survival rate was significantly higher in HPV-positive patients (۶۵%) compared to HPV-negative patients (۴۵%). Patients with HPV-۱۶ had the highest survival rate (۷۰%), followed by HPV-۱۸ (۶۰%), and other types (۵۰%). Tumor recurrence was observed in ۲۰ HPV-positive patients and ۳۰ HPV-negative patients, with a higher recurrence rate in HPV-negative individuals. Metastasis was noted in ۱۰ HPV-positive patients compared to ۲۵ HPV-negative patients. Conclusion The type of HPV infection plays a critical role in determining the clinical outcomes of patients with laryngeal cancer. HPV-positive patients, particularly those with HPV-۱۶, exhibit better survival rates and lower recurrence and metastasis rates compared to HPV-negative patients. These findings highlight the importance of HPV typing in the management and prognostication of laryngeal cancer. References: ۱.
