Introduction: Colorectal cancer (CRC) is one of the most common cancers in the world, and it is one of the leading causes of cancer-related death. Mesenchymal stem cells (MSCs), a unique cell population distinguished by its potential to self-renew indefinitely, develop into several cell lineages, and generate clonal cell populations, have enabled innovative cancer therapy options. MSCs use paracrine signaling to secrete soluble substances called "secretomes," which comprise cytokines, chemokines, extracellular vesicles, and growth factors. These molecules have the same effect as MSCs and also have significant repairable benefits, low immunogenicity and targeted delivery. For regenerative drug therapies, stem cell secretion has great potential in a variety of clinical applications, including functional regeneration in oncological disease. Methods: In this review study, available researches in databases such as ISI, PubMed, Scopus, Google Scholar, ISC, and SID have been used according to keywords. Results: In this study, we have a general discussion about stem cell secretion. Then recent discoveries about the effect of MSCs secretome in the treatment of CRC are presented, and a summary of the inhibitory as well as promoting role of these cells in CRC progression is presented. Conclusion: MSCs secrete soluble factors that regulate the growth and survival of CRC, regulate migration mechanisms, and stimulate receptor immune modulation through paracrine and autocrine functions.
