Persistent infection of the cervical epithelium with human papillomavirus (HPV), coupled with the constitutive expression of viral oncogenic proteins, is established as the primary driver of the complex molecular alterations that culminate in the malignant transformation of cervical epithelial cells. Concurrently, the aberrant regulation of non-coding RNA species_x۰۰۱۰_۱namely microRNAs (miRNAs), long non-coding RNAs (lncRNAs), and circular RNAs (circRNAs)_x۰۰۱۰_۱critically influences the initiation and progression of cervical oncogenesis. Expression profiling of small RNAs in cervical neoplasia has identified the upregulation of key oncogenic miRNAs (such as miR-۱۰a, miR-۲۱, miR-۱۹, and miR-۱۴۶a) and the downregulation of tumor-suppressive miRNAs (including miR-۲۹a, miR-۳۷۲, miR-۲۱۴, and miR-۲۱۸), both of which drive cell proliferation, malignant transformation, migration, and invasive capacity. Similarly, various lncRNAs, including HOTAIR, MALAT۱, GAS۵, and MEG۳, have been implicated in driving tumor progression, metastasis, and therapeutic resistance, highlighting their potential utility as diagnostic and prognostic biomarkers. Furthermore, host-derived circular RNAs, such as hsa_circ_۰۰۱۸۲۸۹, function as molecular sponges to sequester specific miRNAs, thereby disrupting the regulatory networks of downstream target genes. Additionally, the viral-derived circE۷ has been shown to enhance the translation of the E۷ oncoprotein, directly contributing to cellular transformation. In this review, we synthesize recent literature detailing the complex interplay among miRNAs, lncRNAs, and circRNAs, and elucidate their contributions to the pathogenesis of cervical neoplasia.
