Background: Optimal exercise training prescription requires an individualized approach with appropriate balance between stimulus and recovery, supported by effective periodization strategies. Recovery from exercise involves integrated physiological responses, with the cardiovascular system playing a fundamental role in thermoregulation and nutrient/waste product delivery. Cardiac parasympathetic reactivation following exercise has emerged as a valuable marker of cardiovascular recovery that parallels acute and intermediate recovery of thermoregulatory and vascular systems. Despite growing interest, the physiological mechanisms underlying parasympathetic reactivation remain incompletely understood, and practical application for training prescription requires further elucidation. Methods: This comprehensive review quantitatively analyzes post-exercise cardiac parasympathetic reactivation in athletes and healthy individuals following aerobic exercise. The influence of exercise intensity, duration, and fitness/training status on recovery kinetics is systematically evaluated. Evidence from studies utilizing heart rate variability measures to assess parasympathetic recovery is synthesized. Practical recommendations for structuring training microcycles based on parasympathetic recovery kinetics are developed. Results: Metaboreflex stimulation appears to be a key determinant of parasympathetic reactivation in the short term (۰–۹۰ minutes post-exercise), while baroreflex stimulation likely mediates reactivation in the intermediate term (۱–۴۸ hours post-exercise). Complete cardiac autonomic recovery requires up to ۲۴ hours following low-intensity exercise, ۲۴–۴۸ hours following threshold-intensity exercise, and at least ۴۸ hours following high-intensity exercise. Exercise duration is unlikely to be the greatest determinant of recovery. Cardiac autonomic recovery occurs more rapidly in individuals with greater aerobic fitness. Limited data suggest parasympathetic reactivation does not coincide with recovery of all physiological systems, highlighting the need for comprehensive monitoring approaches. Conclusion: Data on cardiac parasympathetic activity, in conjunction with daily training logs, provide valuable information for individualizing training programs. Coaches should structure training microcycles tailored to each individual's unique recovery kinetics, adjusting training intensity and volume based on parasympathetic recovery status to optimize adaptation and performance.
