Background: The intermittent fasting of Ramadan presents unique challenges for observant athletes, potentially affecting various aspects of body physiology and biochemistry important to athletic success. Sleep patterns, circadian rhythms, nutritional intake, hydration status, and psychomotor performance may all be impacted during this month-long period. Despite growing recognition of these challenges, much of the available information has been collected from sedentary subjects or low-level competitors, often without well-matched controls, limiting the applicability of findings to elite athletic populations. Comprehensive understanding of Ramadan's effects on athletic performance and practical strategies for mitigation is essential for supporting observant athletes. Methods: This comprehensive review synthesizes evidence from exercise physiology, sports nutrition, and sleep medicine to examine the effects of intermittent fasting during Ramadan on athletic performance. Physiological parameters including hydration status, energy balance, substrate metabolism, and circadian rhythm disruption are systematically evaluated. Psychological and psychomotor effects including fatigue, vigilance, and reaction time are critically examined. Practical strategies for minimizing negative impacts are synthesized from available evidence. Results: Daytime fluid depletion is inevitable with exercise in the heat, though deficits can typically be replenished at night. Performance of repeated anaerobic exercise is impaired, while aerobic power and muscular strength show minimal change during Ramadan. Ratings of fatigue are increased, and vigilance and reaction times are impaired, particularly during afternoon hours. Top athletes can maintain training throughout Ramadan with appropriate adjustments to scheduling and nutrition. Strategic measures including optimized mood state, maintained training, minimized sleep loss, dietary adjustments, and monitoring for dehydration can help mitigate effects. The impact on performance varies widely with sport type, season, local culture, and individual discipline. Conclusion: The impact of Ramadan upon athletic performance is relatively small compared to test precision, yet sufficient to potentially influence competitive outcomes. Logical measures including optimized mood state, maintained training, minimized sleep loss, dietary adjustments, and dehydration monitoring are recommended. Future research should focus on elite athletes, particularly women, with data collected under controlled conditions to better inform evidence-based guidance.
