Background: Inter-individual differences in size, maturity status, function, and behavior among youth of the same chronological age have long been recognized as a concern in grouping for sport. The mismatch between biological and chronological age can create substantial disparities in physical capabilities, potentially influencing selection processes, injury risk, training responses, and long-term athlete development outcomes. Bio-banding represents a contemporary approach to accommodate maturity-associated variation among youth in sport, yet the historical foundations and current evidence base for this practice require comprehensive examination. Methods: This review synthesizes evidence from pediatric exercise science, growth and maturation research, and talent development literature to examine the historical basis of maturity-matching and its relevance to youth sport. Current bio-banding applications are critically evaluated, with particular attention to assessment methods, implementation strategies, and evidence from applied settings. The percentage of predicted young adult height at the time of observation is examined as the primary estimate of maturity status. Results: Maturity matching in sport has been frequently noted historically but has not been systematically applied until recent iterations of bio-banding. Applications in youth soccer have demonstrated positive responses from players and coaches, with bio-banding reducing but not eliminating maturity-associated variation. Potential utility for appropriate training load prescription, injury prevention, and fitness assessment merits closer attention, particularly during the interval of pubertal growth. The currently used height prediction equation requires further evaluation to ensure accuracy across diverse populations and contexts. Conclusion: Bio-banding offers a promising framework for addressing maturity-associated variation in youth sports, with potential benefits for athlete development, injury prevention, and competitive equity. Successful implementation requires refined assessment methods, sport-specific adaptations, and careful communication with stakeholders. Future research should evaluate the currently used height prediction equation, investigate long-term outcomes across different sports and cultural contexts, and examine the potential for integrating additional maturity indicators.
