Background: The Men's ۲۰۲۶ FIFA Football World Cup will be hosted across the USA, Mexico, and Canada, spanning approximately ۴۳۰۰ km east–west and ۴۰۰۰ km north–south. This unprecedented geographical distribution exposes players to environmental challenges that can negatively impact health and performance, including extreme heat, altitude, air pollution, seasonal allergens, and extensive travel. Extreme heat is expected in ۱۴ of ۱۶ host cities, with historical wet-bulb globe temperatures ranging from ۲۱ to ۳۵ °C. Matches in Guadalajara (۱۵۶۶ m) and Mexico City (۲۲۴۰ m) will occur at altitude. Travel requirements and increased human activities during mega-events elevate susceptibility to airborne illnesses. Air pollution and allergen variations between host cities further threaten player health. Circadian misalignment from crossing up to ۱۹ time zones and travel fatigue from ۷-hour in-tournament flights may impair mental and physical performance. Methods: This review presents predicted environmental challenges using the best available data and models, synthesizing evidence from environmental physiology, sports medicine, and travel medicine literature. Evidence-based long- and short-term strategies were developed to mitigate environmental impacts. Relevant clinical considerations for each challenge were outlined, emphasizing practical applicability within tournament constraints. Results: The combination of extreme heat, moderate altitude, variable air quality, and substantial travel creates a multifaceted threat to player health and performance. Heat stress increases risk of exertional heat illness and impairs endurance capacity. Altitude exposure reduces oxygen availability, affecting aerobic performance. Air pollution exacerbates respiratory conditions and systemic inflammation. Travel fatigue and circadian disruption compromise recovery, cognitive function, and physical readiness. Evidence-based countermeasures include heat acclimation protocols, hydration strategies, altitude pre-acclimatization, air quality monitoring, sleep hygiene optimization, and strategic travel planning. Conclusion: Teams adopting evidence-based guidelines to mitigate environmental challenges will be best prepared to protect player health and performance. Implementation of integrated strategies addressing all identified environmental factors is essential for optimizing outcomes. Future research should evaluate intervention effectiveness across diverse environmental conditions.
