Physical disabilities increase in prevalence with advancing age as if rehabilitation reinstates functionality, prolonging autonomy. Nonetheless, the inadequate availability and accessibility of rehabilitation constrain its clinical efficacy. Interventions led by Artificial Intelligence (AI) have enhanced numerous healthcare areas, although AI's potential benefits in rehabilitation remain ambiguous. We performed a systematic evaluation of AI-assisted physical rehabilitation technology in clinical settings to ascertain the availability of such technology, its clinical efficacy, and the obstacles and enablers to implementation. AI solutions encompassed categories: application-based systems, robotic devices that substitute function, robotic devices that rehabilitate function, gaming systems, and wearable technology. We identified randomized controlled trials (RCTs) that assessed outcomes concerning physical function, activity, pain, and health-related quality of life. The clinical effects were variable. Implementation obstacles encompassed technological literacy, reliability, and user fatigue. Facilitators comprised enhanced access to rehabilitation programs, remote progress monitoring, less labor needs, and reduced costs. The utilization of AI in physical rehabilitation is an expanding domain, yet its clinical impacts require thorough investigation. Developers must endeavor to perform comprehensive clinical evaluations in real-world contexts and assess post-implementation experiences.
