Abstract / Description:
Background & Objective: Platelet-rich plasma (PRP) and absorbable poly-L-lactic-acid (PLLA/PLGA) suspension threads are individually utilized in cosmetic dermatology. This narrative review evaluates the biological rationale, available clinical evidence, and limitations of combining PRP with PLLA/PLGA suspension threads for facial skin rejuvenation.
Biological Rationale: PRP releases autologous growth factors and signaling molecules that support tissue remodeling, whereas PLLA/PLGA suspension threads offer immediate subdermal mechanical support and stimulate local neocollagenesis. However, biological plausibility alone does not confirm clinical synergy or enhanced safety.
Clinical Evidence Analysis:
PRP Alone: Evidence indicates modest improvements in specific skin texture parameters, but randomized trials show limited overall superiority over controls.
PLLA Threads Alone: Studies confirm mechanical tissue repositioning and collagen induction, but report notable complication rates (e.g., dimpling, asymmetry, extrusion, hematoma, and infection).
Combined Therapy: No high-quality controlled clinical trials support incremental efficacy, optimal treatment sequencing, or superior safety when adding PRP to PLLA suspension threads for photoaged skin.
Critical Finding / Retraction Notice: A directly relevant study evaluating PRP combined with PLLA threads in atrophic scars was formally retracted in February 2023 due to procedural/data integrity concerns and cannot be cited as valid evidence.
Conclusion: The clinical efficacy, synergy, safety, and longevity of combined PRP and PLLA thread treatment remain unproven hypotheses. Rigorous prospective randomized controlled trials (RCTs) are required before this combination can be claimed as an established clinical intervention.
