Comparison of Diagnostic Value of ELISA Brucella-IgG and IgM Versus Serologic Tests of Brucella Including Wright, Coomb s Wright and 2ME

Introduction: Brucellosis is one of the most common zoonotic diseases that are endemic in Iran, especially in Hamadan province. Different diagnostic methods including blood culture, serology, capture test, PCR and ELISA are used with different sensitivity and specificity in this regard. Considering the temporary limitations of access to valid serologic diagnosis tests of brucellosis, this study was conducted to compare the diagnostic value of ELISA and serologic tests.Materials and Methods: In this cross-sectional study, by convenience simple sampling methods a total of 231 patients with clinical symptoms of brucellosis referred to Infectious Diseases Clinic of Hamedan Sina Hospital were enrolled in 2018. Patients serum was examined simultaneously with serological tests of Wright, Coomb s Wright and 2ME, as well as brucella-IgG and brucella-IgM by ELISA method. The results of the tests were analyzed with SPSS version 16 at 95% confidence level.Results: Of 231 suspected Brucella patients with mean age of 44.60±16.16 years and 63.64% male gender, 76.6% were diagnosed as Brucellosis based on 1/80 cut-off point of Wright test according to clinical symptoms. Based on IgG and IgM brucella test results, 80.1% and 30.30% of patients were positive for brucellosis, respectively. The sensitivity and specificity of brucella-IgG in comparison to Wright, Coomb s Wright and 2ME were between 83% to 94% and 20% to 33%, respectively, based on different cut off points of serologic tests. Also. the sensitivity and specificity of brucella-IgM in comparison to Wright, Coomb s Wright and 2ME were between 20% to 33% and 78% to 89%, respectively, based on different cut off points of serologic tests.Conclusion: Compared with brucellosis diagnostic serological tests, brucella-IgG has an adequate sensitivity and brucella-IgM has an appropriate specificity. So, combination of these ELISA-tests can be used with sufficient power to detect brucellosis, in the absence of serologic tests. However, due to lower diagnostic value, total replacement of serological tests, doesn t seem to be logical.
