Intravenous versus oral iron supplementation during pregnancy: a review article

intravenous (IV) and oral iron supplements are the main therapies for iron deficiency anemia (IDA), but there is still a debate about their efficacy and especially as to which one is the best choice during pregnancy. The aim of this narrative review is to summarize information regarding the treatment of IDA in pregnancy with intravenous or oral iron supplements. We searched Ovid Medline, Embase, SCOPUS from onset to January 2019. 20 randomized control studies comparing IV to oral iron treatment for IDA are included.All reviewed studies suggest that IV iron administration is effective in treating the IDA. In other words, all forms of IV iron, resulted in improvements in haematological parameters but there is no clear evidence that administration of IV iron is associated with improved clinical outcomes. Therefore, there is an urgent need for studies addressing the safety of IV iron administration during pregnancy in risk of infection, gestational diabetes and preeclampsia. Oral iron is associated with a high rate of gastrointestinal side effects and impact on adherence to oral iron therapy. On the other, Comparisons among intravenous iron supplements show advantages of ferric carboxymaltose (FCM) more than others in time of improvements in haematological parameters and in adverse reactions. Not only, FCM have the ability to administer a high dose (maximum1000 mg), but also over a short period of time (15 min) infusion can be done. Taken together, risk of adverse reactions of IV iron remain unclear whether this is highlights an area for future research.
