Ultrasound Visceral Fat Thickness Correlation with Metabolic Syndrome Components in Non-Obese Pregnant Women with Assisted Conception

Objective: Ultrasonography is a noninvasive and safe modality for assessing body fat and it is performed in almost developed countries as a routine method. Although pregnant women with obesity are at risk for many unfavorable outcomes but, the relationship between abdominal fat distribution and metabolic syndrome is obvious in some studies. Therefore that is important to evaluate fat thickness in no obese women. Considering that MS has especially high prevalence in women and also it is related to unfavorable outcomes in pregnancy, so we planned to this study to simple and early diagnosis of MS through fat thickness measurement.Method and Materials: This cross sectional prospective study was conducted at Royan Institute,Tehran,Iran, between June 2015 to June 2017. A total of 132 pregnant women were completed anthropometric and demographic questionnaires at 11-14 weeks of gestational age. All women were scanned for VFT via ultrasound at 11-14 weeks of gestational age. BMI and WC were calculated at the first prenatal visit. Also FBS, total cholesterol (TC), low-density lipoprotein (LDL), high-density lipoprotein (HDL), triglyceride (TG), and insulin levels were measured at 11-14 weeks of gestational age.Results: Metabolic syndrome was detected in 7 (4.50%) of 132 pregnant women. There was a statistically significant difference between women with MS and without MS for variables (P<0.05). The correlations between MS components and both ultrasonography and anthropometric measurements were evaluated. ROC curve analysis was used to determine the cutoff value of MS. Multiple linear regressions were used for estimating prediction model. Results showed that VFT and WC is effective on insulin and HOMA_IR (P<0.05). The optimal cutoff points for predicting disorders of metabolic syndrome were anterior VFT: 43.83mm and posterior VFT: 32.50mm in this study.Conclusion: Fat thickness measurement in first trimester is a good predictor for MS even in women with normal BMI and ultrasonography as a safe, simple and cost-effective modality can be used for assessing of fat thickness besides the other screening evaluations in first trimester of pregnancy.
