چکیده مقاله:

risk assessment is a logical method to determine the quantity and quality of risks, and alsoto review the potential consequences of possible events on individuals, materials,equipment, and environment. In fact, through risk assessment, the efficiency level ofexisting control methods is specified and valuable data would be provided to makedecisions about reduction of risks and optimization of control systems. Nowadays, themedical staff errors is a serious problem threatening the health system and there arenumerous threats that are effective in occurrence of medical errors, and cannot berecognized easily. It is mandatory for hospitals medical groups to prevent such medicalerrors through mapping work process and identification of hidden threats and potentialerrors by failure mode & effect analysis method (FMEA).this is a quantitative-qualitative research of focus discussion group type (FDG)performed through direct observation, interview with persons in charge and processowners, and studying available documents in different units of Razavi subspecialtyhospital in 2015. Research population includes process owners and those who are familiar with above-mentioned units. Research population includes process owners and those who are familiar with the procedures of mentioned sections and units. Purposive sampling method was used and FDG members were selected by persons in charge for sections and units. FMEA technique was used in this research and information was collected through a standard worksheet which has been used by many researchers.Findings:the results indicate that among 40 detected clinical errors, the highest score belongs to Respiratory Care domain related to error in alarming system of ventilator device and non-compliance of devices settings with doctor s recommendations with 48 scores, and the lowest score belongs to Physiotherapy care related to insufficient education to the patient in Physiotherapy with 10 scores.Conclusion:identification of 6 unacceptable error out of 40 modes of identification error, finding the reasons behind them and performing corrective actions indicate the high capacity of FMEA in identification, assessment, and analysis of possible errors in a complicated and sensitive section such as intensive care unit. Moreover, plenty of errors identified by group members are preventable, and risk management requires more control in health care process in hospital; so that the efficiency and quality of cares improve. Hospital s attention to the position of risk management for improvement of treatment quality and creation of a safe environment for employees and patients are very important. Policies and programs to train and monitor risk management activities in hospitals should be seriously followed.
