چکیده مقاله:
Background: Type ۲ diabetes mellitus (T۲DM) is one of the most prevalent metabolic disorders worldwide and is characterized by impaired glycemic control and dyslipidemia, both of which substantially increase the risk of cardiovascular disease. Resistance training and herbal supplements such as garlic (Allium sativum), particularly its bioactive compound allicin, have each demonstrated beneficial effects on glycemic regulation and lipid metabolism. However, the potential synergistic effects of combining these interventions have received limited attention. Objective: This study was designed to investigate the effects of ۱۰ weeks of resistance training combined with garlic (allicin) supplementation on the lipid profile, including total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), triglycerides (TG), and fasting blood glucose (FBG) in middle-aged men with type ۲ diabetes mellitus. Methods: In this randomized controlled clinical trial, ۴۸ middle-aged men (۴۵–۶۰ years) with type ۲ diabetes mellitus were randomly assigned to one of four equal groups (n = ۱۲ per group): (۱) resistance training plus garlic supplementation, (۲) resistance training plus placebo, (۳) garlic supplementation alone, and (۴) control (placebo without exercise). Participants in the training groups performed a progressive resistance training program three times per week for ۱۰ weeks. Participants assigned to the supplementation groups received ۶۰۰ mg/day of standardized garlic powder, equivalent to ۳.۶ mg of allicin, throughout the intervention period. Fasting venous blood samples were collected at baseline and after the ۱۰-week intervention to determine serum concentrations of total cholesterol, LDL-C, HDL-C, triglycerides, and fasting blood glucose. Results: After ۱۰ weeks, the resistance training plus garlic supplementation group demonstrated significant reductions in fasting blood glucose (P < ۰.۰۰۱), total cholesterol (P < ۰.۰۰۱), LDL-C (P < ۰.۰۰۱), and triglycerides (P = ۰.۰۱), along with a significant increase in HDL-C (P < ۰.۰۰۱) compared with the control group. Furthermore, significant synergistic effects were observed between resistance training and garlic supplementation for improvements in fasting blood glucose (P = ۰.۰۱) and total cholesterol (P = ۰.۰۵). Conclusion: Ten weeks of resistance training combined with garlic (allicin) supplementation produced synergistic beneficial effects on glycemic control and lipid profile in middle-aged men with type ۲ diabetes mellitus. These findings suggest that this combined intervention may represent an effective non-pharmacological strategy for the management of patients with type ۲ diabetes mellitus.
