چکیده مقاله:
Venous thromboembolism remains an important cause of maternal morbidity during pregnancy, with deep vein thrombosis (DVT) occurring more frequently in the third trimester because of physiological hypercoagulability and venous stasis. Prompt diagnosis and early anticoagulation are critical to prevent thrombus extension and potentially fatal complications such as pulmonary embolism. A ۲۶-year-old gravida ۲ para ۱ woman at ۳۶ weeks and ۵ days of gestation presented with progressive pain, swelling, and erythema of the left lower limb. Doppler ultrasonography confirmed a ۲۲-mm venous thrombus in one leg, and the patient was admitted to the intensive care unit for further management. Her medical history was notable for gestational diabetes mellitus treated with metformin and beta-thalassemia minor. Laboratory evaluation revealed hemoglobin of ۹.۸ g/dL and platelet count of ۱۲۶,۰۰۰/mm³. Therapeutic anticoagulation with intravenous DRIP of heparin was initiated, and close maternal-fetal monitoring was maintained throughout hospitalization. Doppler ultrasonography was repeated after five days demonstrated no resolution of the thrombus. On the seventh hospital day, spontaneous labor occurred and the patient subsequently underwent an uncomplicated vaginal delivery. This case demonstrates that timely diagnosis and appropriate use of heparin can result in favorable maternal and neonatal outcomes in pregnancy-associated DVT.
