This study aimed to conduct a comprehensive evaluation of the Health, Safety, and Environment (HSE) management system at Fatemeh Al-Zahra (SA) Specialized Hospital during the second half of ۲۰۲۵ (Iranian calendar: second half of ۱۴۰۴). Employing a descriptive-analytical approach and a case study methodology, this research examined the current status, hazard identification, risk assessment, and effectiveness of control measures within the hospital. The study population consisted of ۷۵۰ hospital staff members. Data collection instruments included HSE checklists, risk assessment forms (FMEA and JHA), semi-structured interviews, and document reviews. The results indicated that the hospital’s HSE system was highly effective in controlling technical and facility-related hazards, including tank explosions (RPN reduction from ۲۸۰ to ۶۰), fire (from ۲۱۶ to ۵۴), and electric shock (from ۳۳۶ to ۴۲), such that the mean RPN decreased from ۱۸۷.۰ to ۳۵.۹ (a ۷۸.۲% reduction), and this reduction was statistically significant (p < ۰.۰۰۰۱). Furthermore, the hospital possesses robust safety infrastructures, including complete grounding system coverage and good earthquake preparedness. However, needlestick injuries (accounting for ۵۳% of all incidents) and biological hazards remain the primary challenges. Key recommendations include strengthening needlestick injury prevention programs, placing greater emphasis on biological hazards, completing safety standard coverage, and implementing root cause analysis of incidents. This study underscores the importance of systemic approaches to HSE management and the necessity of continuous attention to clinical and biological aspects.
