Antibiotic therapies, while essential, often cause profound collateral damage to the commensal gut microbiota, leading to dysbiosis, impaired colonization resistance, and associated clinical sequelae such as antibiotic-associated diarrhea (AAD). This review delineates the transformative role of nanotechnology in developing advanced delivery systems designed to protect and synergistically enhance the efficacy of microbiome-based therapeutics. We critically examine Nano encapsulation strategies that utilize pH-sensitive polymers, polysaccharide matrices, and single cell Nano armors to shield probiotics from gastric acidity, bile salts, and concomitant antibiotics, thereby drastically improving their survival and viable delivery to the colon. Furthermore, the review explores the engineering of targeted Nano prebiotic systems for the selective stimulation of beneficial bacterial growth and the design of nanosynbiotics that co-deliver probiotics and prebiotics for synergistic action. Key mechanisms include stimuli-responsive release (pH, enzyme, microbial metabolite-triggered), enhanced mucosal adhesion via functionalized Nano films, and the creation of protective local microenvironments. We also evaluate the multifunctional biological activities of these nanoprobiotics, including their antimicrobial, antioxidant, immunomodulatory, and barrier restoration properties. Despite significant promise, translational challenges concerning long-term stability, scalable GMP manufacturing, and precise in vivo targeting remain. This review concludes by advocating for the rational design of next-generation, personalized Nano-biotherapeutics to achieve rapid and resilient restoration of a healthy microbiome following antimicrobial interventions.
