Assessing the effect of hepatic steatosis on liver stiffness measurement in HIV-positive individuals

Introduction: Fatty liver disease is increasingly becoming recognized as a worldwide health problem. Inflammation caused by varying degrees of fat accumulation in the liver tissue is the disorder's hallmark. The combination of viral hepatitis and fatty liver is notable since research has shown that fatty liver is a significant factor influencing the prognosis of individuals with chronic hepatitis. A helpful method for determining and quantifying the degree of steatosis is transient elastography. If the liver stiffness measurement (LSM) index has a high diagnostic value in detecting hepatic steatosis, it may be a suitable alternative for these individuals. Methods: An analytical cross-sectional examination of ۵۳ hepatitis B patients (۲۶ men and ۲۷ women) who received treatment at Ahvaz, Iran's Aria Hospital, between April ۲۰۲۳ and November ۲۰۲۴ is presented in this pilot research. ۳۴ patients were placed in the therapy group, while ۱۹ individuals were placed in the non-treatment group. The Liver Stiffness Measurement (LSM) index and transient elastography were used to assess the participants' prevalence of hepatic fibrosis. LSM was used to do further comparisons between individuals who received therapy and those who did not. Results: The untreated group had a ۲۵% prevalence of steatosis, whereas the treated group had a ۲۶.۷% prevalence. The incidence of fibrosis was ۵۷.۹% in the untreated group and ۵۸.۸% in the therapy group. The relationships between the study variables and the LSM index were assessed in both the treatment-treated and non-treated groups of hepatitis B patients. All of the correlations were statistically significant. The diagnostic utility of the LSM for hepatic steatosis was also assessed. While the test's diagnostic value was good in the untreated group, it was restricted in the therapy group. Conclusions: The LSM obtained from transient elastography in Hepatitis B patients did not exhibit a statistically significant relationship with variables like age, gender, body mass index (BMI), or the severity of hepatic steatosis. This research found that, when compared to the CAP score (AUC = ۰.۶۹ and ۰.۷۴), the LSM number is not a valid diagnostic for detecting hepatic steatosis in hepatitis B patients.
