One of the most commonly used probiotics is Lactobacillus casei, which has significant implications for human health. Having a high resistance to gastrointestinal diseases, low immunogenicity, and the ability to alter gut microbiota, this bacterium has generated interest as a primary and adjunctive therapeutic intervention in infectious diseases, metabolic diseases, and inflammatory diseases. This narrative review involved the search of the Google Scholar, PubMed, and SID databases and the extraction of reputable sources on the antibacterial activities of this probiotic until ۲۰۲۵. Increased mucosal immunity, a decrease in intestinal inflammation, and a better composition of microbiota have been clinical and lab-based research findings associated with the consumption of L. casei. Lactobacillus casei antibacterial mechanisms generate lactic acid and hydrogen peroxide, generate bacteriocin, outcompete the pathogens in binding to the intestinal epithelial surface, and stimulate the immune system in the host. These antimicrobial effects have been exhibited in drug-resistant agents like E. coli, K. pneumoniae, Helicobacter pylori, and Pseudomonas aeruginosa. Recent reports also indicate that L. casei can also have antibiofilm, and antioxidant properties. Taken together, these data show that L. casei is an option that is safe and effective to increase the health of humans and that it can be used as a possible medicinal supplement for a large number of diseases.
