Schema Therapy (ST), developed by Jeffrey Young, is an integrative psychotherapeutic approach that addresses deeply ingrained Early Maladaptive Schemas (EMSs) originating from unmet core emotional needs during childhood. These pervasive and rigid patterns of thinking, feeling, and behaving are hypothesized to underlie various psychological difficulties, particularly chronic and challenging conditions such as personality disorders (PDs). This article offers a comprehensive exploration of the theoretical foundations of Schema Therapy, including its conceptualization of EMSS, maladaptive coping styles, and schema modes. It critically reviews the robust empirical efficacy of ST in remediating these early maladaptive patterns across different personality disorders. Drawing primarily from recent randomized controlled trials (RCTs) and comprehensive meta-analyses (e.g., Renner et al., ۲۰۲۱; Spinhoven et al., ۲۰۲۲), particular attention is given to its well-established effectiveness in treating Borderline Personality Disorder (BPD). The article also highlights emerging evidence for its applicability and positive outcomes in other PDs, including those in Cluster C and Narcissistic Personality Disorder (Bamelis et al., ۲۰۱۴; Rees et al., ۲۰۲۱). The discussion delineates the proposed mechanisms of change within ST, such as limited reparenting and experiential techniques, and examines the long-term sustainability of treatment gains (Giesen-Bloo & Arntz, ۲۰۱۸), along with practical implications for clinical practice and societal cost-effectiveness (Van Asselt et al., ۲۰۰۸). This review underscores Schema Therapy's vital role as a potent and transformative intervention capable of fostering profound and lasting psychological change in individuals struggling with complex and enduring personality pathology.
