Background and Aims: In the recent decades, the integration of robotics with artificial intelligence (AI) has actually transformed the realm of rehabilitation. The technologies support customized training, intelligent motion tracking, and real-time bio-signals processing, which contribute to improved motor rehabilitation of patients with various physical disabilities. In this review, efforts have been made to tackle the applications, trends, and challenges of rehabilitation robots with AI-based in various clinical conditions. Methods: In this narrative review, an extensive search was conducted in the PubMed database and Google Scholar search engine using English keywords including artificial intelligence, rehabilitation, rehabilitation robots. ۸۷ articles were found. After removing duplicates, outdated studies, and irrelevant content, ۱۳ articles published between ۲۰۲۰ and ۲۰۲۵ were selected for analysis and review. Results: Rehabilitation robots with AI have shown effectiveness in enhancing motor function, especially in stroke, Parkinson's disease, and rehabilitation of children. EMG signal-based and wearable sensor-based systems identify patient intent and offer adaptive upper limb rehabilitation assistance. AI-controlled exoskeletons and vision-based devices enable real-time feedback and personalized guidance, even in outpatient settings. With promising outcomes and increased patient engagement through VR and serious games, there are however issues of clinical validation and accessibility. However, AI continues to revolutionize rehabilitation with smarter, more responsive technology. Conclusion: AI rehabilitation robots take center stage in the optimization of motor function, customization of treatment, and enhancing the efficacy of rehabilitation processes. Despite great strides, issues such as requirements for clinical validation, exorbitant costs, and low levels of professional adoption still exist. Future developments of the technology include designing light, accurate, and cost-effective systems that will be suitably established in rural and everyday healthcare services.
