Introduction: The health system faces several challenges that affect its ability to provide quality care to patients. These challenges include poor system design, inability to respond to changing patient demographics, slow adoption of information technology innovations, personnel shortages, and poor working conditions. Additionally, the health care system is inadequate in meeting the needs of the chronically ill. This review article aims to classify the challenges of health systems investigated in the scientific literature. Materials and Methods: In this review, using appropriate keywords, search engines and scientific databases including Google Scholar, Magiran, SID, Iranmedex, PubMed, and Irandoc were searched. Studies published in Persian and Latin databases with keywords: 'health system challenges', 'educational challenges', 'research challenges', 'economic challenge of the health system' according to Mesh terms and combining synonyms with 'AND' operators. 'OR', 'NOT' were selected in the period of ۲۰۲۱ to ۲۰۲۳. After reviewing and evaluating the titles and abstracts according to the acceptance criteria, among the ۳۴ studies conducted, ۱۳ studies were selected for review. Findings: This survey showed that the challenges facing the health system are divided into three categories: the macro challenges of the health system, the systemic context, the micro challenges of the health system, the organizational context and the micro context related to the individual health care manager. The cost of health care is a burden on families, and health care costs play a significant role in decisions about insurance coverage and care seeking. Conclusions: The challenges facing the health system are complex and continually changing across a variety of contexts and health service levels. The health system needs to adapt to epidemiological, demographic, and societal shifts. Emerging technologies and political, economic, social, and environmental realities create a complex agenda for global health. Health leadership and workforce management need to prioritize addressing these challenges to provide quality care to patients.
