Effective factors on visual complications in type ۲ diabetes patients by survival analysis methodin Zahedan ۲۰۲۲

Introduction: Diabetes as a metabolic, endocrine glands disorder is a public health problem in the world andis the most common cause of blindness, amputation, chronic kidney failure and heart diseases. More than۹۰%of the diabetes prevalence is type۲, which includes۹% of global diabetes deaths. In Iran, the control status ofpatients with type۲ diabetes is far from optimal and delay in diagnosis, control and lack of knowledge about thecomplication’s occurrence time, increases their incidence risk. Survival analysis is a suitable statistical methodfor predicting the time of occurrenceMethods: This retrospective cohort were studied۴۱۰ patients who had been diagnosed with diabetes of type۲for at least ۵years, referred to the Bo-Ali diabetes clinic in Zahedan city in ۲۰۲۰ year. To record the data analysisusing Stata.ver۱۴, for the effects of demographic factors using the Cox proportional-hazards model, the mediansurvival time (Month)from logrank-test, selection of variables from forward LR-Test method and survivalfunction diagram and Kaplan-Meier diagram were used to better present the complication occurrence time.Proportionality and Log survival charts used for the final model of the independent variable’s coefficients withthe time of vision complications occurrence.Results: In the present study, ۱۰۳people (۲۵.۱۳%) had vision problems, and in relation to this problem, theindependent variables A۱c, HDL and systolic blood pressure remained in the regression model without beingcollinear with each other. An increase of ten units of A۱c increases ۱.۲۱times and an increase of ۱۰units ofsystolic blood pressure increases the chance of vision complications by ۱.۰۲times, and for an increase of tenunits of HDL, the chance of vision complications decreases by۷%. The Log-Rank test showed changes in maritalstatus (OR=۱.۲۲), age (OR=۱.۵۶), irregular visits (OR=۱.۰۶), type of treatment (OR=۲.۶۸), smoking(OR=۲.۶۸)), cholesterol (OR = ۱.۰۰۶), fasting blood sugar (OR = ۱.۳۲) and two-hour blood sugar (OR = ۱) havea significant relationship with the chance of visual complications (P < ۰.۰۵), but Employment (OR=۱.۱۷) andinsurance (OR=۱.۵۲) were not statistically significant(P<۰.۰۵).Conclusion: According to the findings of this research, more than half of the population had at least one of therisk criteria of diabetes complications, which has a significant effect on reducing the complications survivaltime. Prevention and control interventions in postponing the occurrence of risk factors are effective and if peopleare infected, proper care and treatment will increase the life span without complications.
