چکیده مقاله:

Introduction :Aluminum phosphide (AlP) or rice tablets is one of the most common pesticides in the world. This substance releases phosphine gas (PH۳) in the presence of water vapor or stomach acid, which causes poisoning. Poisoning caused by phosphine is more suicidal and its number is increasing day by day, two thirds of the poisoned people die.Purpose:The aim of this study was report of a case of poisoning with Aluminum Phosphide (Rice tablets).Patient introduction:A ۳۰-year-old man goes to the emergency room at night with an initial complaint of consuming rice tablets an hour ago, according to the patient's companion. The initial symptoms of BP=۱۳/۸۰ HR=۱۱۰ GCS=۱۵/۹ in VBG are evident metabolic acidosis. The patient is transferred to the ICU after initial measures in the emergency room. In the initial nursing evaluation, it was found that the patient vomited after eating rice tablets and was immediately taken to this center by the companion. Due to the continuing trend of severe drop in blood pressure and the lack of appropriate treatment response to epinephrine, the GIK protocol is started for the patient, due to the acidosis, he receives sodium bicarbonate. The vital signs of sugar were monitored every hour by the nurse. In the initial EF echo = ۱۵%. With a relative increase in consciousness, the patient complains of thirst and is restless. In order to increase the patient's awareness of the process of self-care treatment, they were taught that there is a positive feedback for the non-absorption of rice tablets through the gastrointestinal tract of a patient who has become NPO. After ۵ days of hospitalization in ICU, due to the continuation of treatment with sodium bicarbonate, GIK protocol and adherence to being NPO by the patient's relatives, he was transferred to the ward with full consciousness in echo EF = ۵۰%. The patient was discharged in good general condition after ۷ days of recovery.Discussion and conclusion:Considering the relationship between the death rate caused by Rice pills and the time between poisoning and the patient's arrival at the treatment center, the number of pills taken and the intake of liquids after poisoning, these criteria can be used as prognosis estimates in This poisoning used. Therefore, public education on self-care has a significant impact on the mortality rate and morbidity caused by it.
