Compared to centralized hemodialysis (HD), peritoneal dialysis (PD) has many benefits that cost-effective and comfort of patients is most important. Many patients prefer the independence it brings. Since they don’t have to have dialysis sessions at a dialysis center, they can give themselves treatments at home, at work or on holiday. On the other hand, receiving peritoneal dialysis requires different services, so the set of these services can be provided as home healthcare (HHC) services for patients in need of (PD). In HHC companies, in addition to reducing total costs, it is very important to pay attention to patient satisfaction to increase the company s profitability. On the other hand, the transportation is identified as the largest source of pollution in any logistic and supply chain systems such as the HHC company so reducing environmental pollution is a goal that is considered in various issues today due to environmental problems. In this paper, for the first time, a model with the aim of social sustainability is presented with three goals: reducing the total costs of the system, reducing environmental pollution and increasing patient satisfaction. To bring the model closer to real world issues, a variety of vehicles and nurses have been considered. There is a hard time window for service time. Patients can also receive four types of services from the institution according to their circumstances. They also have priorities and preferences for choosing a nurse. Finally, the multi-objective model is solved using the Epsilon-constraint method.
