Background and Objective: The main goal of the operating room field is to train experts who can effectively participate as a member of the surgical team(1). The introduction of new teaching methods has created challenges in education, especially in medical sciences(2). One of these techniques is simulation training(3). This study aimed to examine the effect of using Appendectomy Surgical Simulation software (game-based) on Academic Achievement of undergraduate students in operating room in Khomien. Materials and Methods: In this single-blind quasi-experimental study, 50-second semester undergraduate students were selected by convenience sampling. After obtaining satisfaction, they were assigned to intervention and control groups. After using the simulation software (game-based) for the Intervention group and the holding practice and lecture class (routine method) for the control group, the data were collected by a self-administered questionnaire of Academic Achievement which the validity and reliability were confirmed. Data were then analyzed using SPSS software version 16 and descriptive and analytical tests with significance level of 0.05.Findings: The results showed that the average score of Academic Achievement in the intervention group (57/24 ± 1/23) was higher than the control group (39/41 ± 1/87), which was statistically significant (P-value <0.05).Conclusion: The simulation software presented in this article has been effective in improving the academic achievement of students, so the use of this method in education is recommended
