Antimicrobial resistance patterns of Klebsiella pneumonia, isolated from blood cultures in Namazi hospital, shiraz-iran, March 2018- March 2019

Introduction and objective: Klebciella pneumoniae is one of the most important bacteria causing nosocomial infections. On the other hand regardingincreasing antimicrobial drug resistance, is now a global concern around the world, this bacterium threatens public health and leads to increased hospital costs and mortality rate.This study was aimed to performdisk diffusion susceptibility test on the Klebsiella. pneumoniae isolated from blood cultures in PACMRC Shiraz Iran during March 2018- March 2019. Material and Methods: This study was performed on 73 of K. pneumoniae isolated from blood culture samples. Strains were identified at species level byAPI-20E analytical system. Uncertain isolates were confirmed by Phoenix automated microbiology system.The antimicrobial susceptibility testto 25 antibiotics was performed, using Kirby-Bauer disk diffusion test. Results: From 73Klebsiellapneumonia isolates, The highest rate of resistance in isolates of K. pneumoniae was seen against ampicillin (98.6%),Amoxicillin/Clavulanic acid(84.5%), Cephalexin(72.6%) ,Ampicillin/Sulbactam (68.6%),Cefuroxime(65.7%),Cefixime(64.8%),Ceftriaxone(63%),Cefotaxime(63%),Ceftazidime(61%),Aztreonam(61%), Cefepime (58.6%).23.3% were resistant to imipenem,31.9% were resistant to meroponem and 76.9 % were ESBL positive. Conclusion: There is a relatively remarkable prevalence of carbapenem-resistant Klebsiella pneumoniae isolates. Besides, high rate of ESBL positive isolates strongly suggest a necessity for the implementation of effective strategies to prevent and control the spread of antibiotic resistant infections.
