Diagnosis of carbapenemases in Klebsiella Isolates from UTI Patients by phenotypic Tests and PCR

Background and Objectives: Due to the destruction of penicillins, cephalosporins and carbapenems by carbapenemase enzymes that create many problems in treating infectious diseases, the present study was designed to investigate the phenotypic and molecular characterization of carbapenemase enzymes and VIM and IMP genes in Klebsiella isolates in an educational hospital in 2016. Materials and Methods: In this cross-sectional study, 80 urinary tract infection samples caused by Klebsiella were obtained from different parts of the hospital . To identify Metallo-beta-lactamases (MBL) enzyme, phenotypic tests containing Modified Hodg Test, EDS Test and Ampc disk were performed. The frequency of VIM and IMP genes were determined by PCR. Results: According to the results, amongst 80 Klebsiella samples, 24 (30%) were resistant to meropenem and imipenem. Also, phenotypic tests, including, Modified Hodg Test, EDS test and AmpC disk test showed the positivity of 18 (22 . 5%), 18 (22 . 5%) and 10 (12.5%) isolates, respectively. PCR test result for VIM gene was 22 (27 .5%) positive isolated from Klebsiella, but IMP gene was not observed in any of the isolates. Conclusions: VIM and IMP are class B of MBL, and the destruction of penicillins, cephalosporins and carbapenems has created many problems for the treatment of infectious diseases.
