The effect of breastfeeding duration in mother-to-child transmission of human t-cell lymphotropic virus type 1 (HTLV-1); a systematic review

Background: HTLV-1 is the first exogenous retrovirus discovered. Transmission of HTLV-1 trough breastfeeding thought to be the major way of vertical transmission. This study examined the role of breastfeeding in HTLV-1 vertical transmission and effect of breastfeeding duration on it. Research design and Methods: performed a search of PubMed, Cochrane and Science direct database for HTLV-1, breastfeeding, mother-to-child transmission. 276 article were extracted (37 Cochrane, 102 PubMed, and 137 Science direct) and according to this prism chart, 11 articles were included. Results: important risk factors in MTCT were maternal proviral load (P = 0.005), sibling with HTLV-1 (P < 0.001) and duration of breastfeeding (P < 0.001).in this systematic review concentrated on a duration of breastfeeding. The results of two studies presented in the table indicated that breastfeeding for 12 months and more was associated with an HTLV-1 seropositive offspring. Conclusion: in this study found the correlation between breastfeeding duration and MTCT of HTLV-1 in previous studies but the number of the studies in this field were very low And requires cohort and cross-sectional studies on seropositive mothers and their children.
