The prevalence of CYP2C9*4 polymorphism in different regions of Iran

Background: Cytochrome CYP2C9*4 is a key enzyme with a major role in the activation and detoxification of many xenobiotics and medicines. The purpose of this study is to determine the genotypic and allelic frequency of this polymorphism in Iranian population and compare with other populations of the world. The results of this study can be used in treatment, prevention of adverse medicine reactions and associated deaths, as well as screening drug interactions by P450 enzyme. Materials and Methods: In this study, 300 blood samples of unrelated and healthy donors from different ethnicities of Iranian population (including Fars, Azari, Caspian (Mazani, Gilaki), Lure and Kurd) which referred to Special Medical Center, Tehran, Iran, were obtained. CYP2C9*4 polymorphism was analyzed by PCR, RFLP and SSCP on collected samples. Results: According to the investigation of CYP2C9*4 allele in 300 blood samples, about 18.7% of cases have a homozygous mutant allele (T/T), 8.3% heterozygous allele (C/T) and 73% wild-type homozygous allele (C/C). The frequency of homozygous CYP2C9*4 allele in different ethnicities is as following: Lurs (16%), Azari (19%), Persians (18%), Kurds (4%), and Mazandaranians (14%). Also, its frequency in different geographic regions of Iran is as follows: North (17.1%), South (10%), East (0%), West (16.5%) and Centre (0%). Conclusion: The frequency of CYP2C9*4 allele has been reported 12-20% in Persian Gulf populations, 15-30% in Iran’s eastern neighbors and 26-40% in Iran s northern neighbors and it differs based on differences in ethnicity and geographical region. According to the results reported in this study and comparison with numerous other countries, it was found that the frequency of CYP2C9*4 allele is more in Iran compared to other countries. Overall, the frequency of CYP2C9 4 allele in Iran is greater than East and West Asia
