Quality and quantity of 2672 kidney needle biopsies, received by electron microscopy unit from Shiraz University of medical sciences affiliated hospitals during 15 years

introduction Several cases of kidney diseases require a direct sample of the kidney to undergo pathological examination for the certain diagnosis. The kidney sampling can identify subtypes of diseases and neoplasms. So it s a useful tool for better diagnosis of kidney disease. The importance of sampling accuracy is not overlooked. Therefore, this study aimed to evaluate the quality and quantity of kidney needles biopsy sent to Shiraz electron microscopy unit from hospitals affiliated to Shiraz University of Medical Sciences from 2001 to 2015. Consequently, the results of this study can provide useful information to relevant specialists and help improve the quality and quality of future kidney biopsies.Materials and method In this cross-sectional study, the electron microscopy reports of all kidney biopsies were sent to electron microscopy unit from shiraz university of medical sciences affiliated hospitals for 15 years (2001-2015) were included in the study. Demographic data, the name of hospitals and wards and final diagnosis (by our electron microscopy) were recorded. The study was approved by the Ethics Committee of Shiraz University of Medical Sciences (code: IR.SUMS.MED.REC.1397.208). Descriptive statistics were reported as mean±SD or frequency (%) for quantitative and qualitative data, respectively. The association between the two categorical variables was assessed using the chi-square or the Fisher’s exact test, where appropriate. Statistical analysis was performed using the SPSS software (version18.0) and P<0.05 was considered statistically significant.Results This study shows that over 15 years, from 2672 kidney biopsies (male/female = 1150/1522) were sent to election microscopy unit, 2309 (86.42%) of them were successful containing an adequate number of glomeruli. So for all of them, a glomerular disease was diagnosed. The main categories of them included Primary glomerulonephritis(n=1770), systemic disease based glomerular diseases(n=560), hereditary(n=23), rejection(n=6), tubulointerstitial disease(n=74), others(n=400). On the other hand, 363 kidney biopsies(13.58%) has been failed and were inadequate for a final diagnosis. They included adipose tissue(3.58%), skeletal muscles(27.82%), only medulla(30.30%) and other(38.30%). Namazee hospital had the most number of the specimen (2028 specimens) followed by Faghihi(n=116).ConclusionBy considering the high failure rate of procedure of kidney biopsies that were sent to electron microscopy unit, we suggest that if the nephrologist examine the kidney biopsy specimen immediately after the procedure with a stereomicroscope to see the dark red dots of glomerulus, the failure rate could be decreased. To the best of our knowledge, this is one of the few large-scale investigations in Iran using electron microscopy.
