Maternal and fetal characteristics based on mode of chorionicity in twin pregnancies

Objective: The aim of this study was to compare the maternal and fetal characteristics based on mode of chorionicity.Materials and methods: We performed a retrospective cross sectional on 124 women with twin pregnancies from April 2014 through July 2017 at perinatal clinic.The variables including maternal age (year), BMI (kg/m2), type of conception (in vitro fertilization, induction or spontaneous), parity, crown-rump length (mm), chorionicity (Monochorionic (MC) or Dichorionic (DC), FHR difference, NT discordance were recorded. Results: Of 124 subjects, 15 women (12.1%) were MC and 106 (85.5%) were DC.. The mean maternal age in MC group were significantly lower than DC group (27.4±5.3 year vs 31.15±5.2 year, respectively, p value= 0.01). The parity did not differ significantly between MC and DC twin pregnancies (P value =0.6). All IVF pregnancies led to DC twins. Type of conception significantly differed between MC and DC twins (P value= 0.006). There was no significant difference in median CRL discordance between MC and DC twin pregnancies (5.1% (IQR, 2.9-10.2%) vs 6.1% (IQR, 2.9-12.5%), P value= 0.5). The inter-twin FHR correlation in MC group was r= 0.2 (P value=0.3) and in DC group was r=0.5 (P value= 0.001). There were differences in NT discordance between MC and DC groups (32 ± 27.6 vs 18.5 ± 15.7, P = 0.01). Conclusions: In our study MC twin pregnancies were happened more in young women & DC twin pregnancies were happened more in older women & women who conceived by in vitro fertilization (IVF), so from this aspect of view , DC twin pregnancies may have some risks more than monochorionic twin pregnancy & might need special prenatal care other than MC twin pregnancies.
