Pethidine as an Opioid for Postoperative Cesarean Pain; Investigation of the Heart Rate and Respiratory Rate in Breastfeeding Infants

Background & Purpose: One of the pharmaceutical methods used to relieve pain after cesarean section is the use of opiates. However, pain relieving can be useful when it does not have bad consequences for the mother and the baby. One of the important issues that should be considered in this regard is the physiological variables of the infant. The present study was conducted to determine the effect of pethidine opioid on heart rate and respiratory rate in breastfeeding infants in mothers receiving pethidine after cesarean section by spinal anesthesia technique. Methods: This clinical trial study was performed on 101 neonates born by cesarean section and spinal anesthesia in Gerash Amiralmomenin Hospital in 2017. Mothers eligible for this study were randomly assigned to the experimental and control groups using binary blocks. The experimental group received 100 mg of pethidine as intravenous infusion and control group received the routine care (diclofenac suppository). Heart rate and respiratory rate were recorded in both groups in the first 48 hours of birth. Data were analyzed using independent t-test, chi-square test and Spss22 software. Findings: The findings of the study showed that there was no significant statistical difference between the two groups (p=0.335) despite the fact that the mean heart rate of the infant in the experimental group was more than that of the control group during the first 48 hours of birth. Moreover, although the mean respiratory rate in the first 48 hours in the control group was higher than that of the control group, statistically there was no significant difference between the two groups (p=0.16).Conclusion: Based on the obtained results, the administration of pethidine (100 mg) in mothers of cesarean section to control pain may not have harmful effects on the heart rate and respiratory rate of breastfeeding infants
