Clinicopathologic evaluation and prognostic analysis in Nonhodgkins lymphoma

Background: Non-Hodgkin lymphoma is heterogeneous disease in pathological evaluation,clinical course, treatment and prognosis. The prevalence of this disease has increased in recentdecades. Despite the importance of this issue today, few research and study methods have been usedto improve treatment and prognosis, So, we decided for evaluate the clinical and pathologicalfactors and the analysis of important prognostic factors, including Para clinical tests in therecurrence of patients with non-Hodgkin lymphoma and response to salvage therapy.Material and methods: This retrospective study was conducted on 166 non-Hodgkin lymphomapatients in health centers of Shiraz University of Medical Sciences. History, positive point ofphysical exam, bone marrow, spleen and extranodal involvement, bulky disease and mediastinalmass are recorded. Types of pathology are classified according to the subgroups of B, cells andIHC and then all patients were classified into two groups: low risk and high risk based on theresponse to primary and salvage therapy. At the end, all patients were assessed with Chi-square forprognostic factors, clinical and pathological symptoms, and response to treatment based on the typeof treatment.Results: 166 patients were enrolled in this study and the mean age was 45.65 16.14 and the ratioof men to women is 1.5: 1. Absence of Bone marrow involvement (P 0.002), normal hemoglobin,platelets count and normal serum LDH level (P <0.05) were associated with lower risk of diseaseprogression. However, symptom, spleen and BM involvement (p:0.0001) and extra nodalinvolvement (P: 0.001) have significant relationship with death. We detect Hb ≤14 in malepatients that was associated with poor response to salvage chemotherapy. The most common pathology in patients was DLBCL with 81.81% followed by follicular and burkitt lymphoma insecond place. Among all immunohistochemistry, patients with positive CD20 and PAX5 (P:0.0001& P: 0.001), had significant relationship with lower risk in patients with NHL.Conclusion: Positive CD20 and PAX5 in patients with non-Hodgkin lymphoma are goodprognostic factors and also multiple clinical and pathologic factors are effective in responding to thetreatment and the course of the disease.
