NUTRITIONAL ASSESSMENT AND ADEQUACY OF DIETARY INTAKE IN INTENSIVE CARE UNIT PATIENTS IN EMAMREZA HOSPITAL, MASHHAAD, IRAN

Background and Aim: Malnutrition is a major problem within intensive care units (ICUs) affecting the outcome of patients. There are numerous reports that the nutritional intake of many hospitalized patients is sub-optimal. The purpose of this study was to analyze the hospital gavage solution which is prepared at hospital kitchen and compare the calculated nutritional demand with the provided values.Methods: In this study,30 ICU admitted patients were included. The required energy (25Kcal/day) and protein (1.2g/day) values for the first three days were compared with the provided values. The amount of carbohydrate, lipid and protein of hospital gavage solutions were determined with Leen ion, Gerber and Kejeldal methods.Results: The carbohydrate, lipid, protein and energy contents of milk and egg cocktail gavage solutions were 0.073g/ml, 0.014 g/ml, 0.019g/ml (27% lower than standard regime),and 0.52Kcal/ml(66% lower than standard regime),respectively and for mixed soup were 0.072g/ml, 0.036g/ml, 0.029g/ml, and 0.73Kcal/ml (52% lower than standard regime), respectively. Mean age of our patients was 54.4±23 years and 50% of them were male. Mean energy requirement and provided at the first three days of admission to ICU was 1569.9±183.7Kcal/day and 456.6Kcal/day, respectively (p<0.001).Mean protein requirement and provided was 70.9±15g/d and 22.5±23.3g/day, respectively (p<0.001).Conclusion: The gavage solution provided by hospitals cannot meet the calorie and protein needs of patients. It is necessary to prepare solutions with enough contents of carbohydrate, lipid and protein or to provide them with standard enteral nutritional solutions.
