FORTIFIED YOGURT WITH BETA-HYDROXY BETA-METHYLE BUTYRATE (HMB), VITAMIN D, AND VITAMIN C IMPROVES MEASURES OF SARCOPENIA IN SARCOPENIC ELDERLY: A DOUBLE-BLIND RANDOMIZED PLACEBO-CONTROLLED TRIAL IN COMMUNITY-DWELLING OLDER ADULTS

Background and Aim: Sarcopenia, age-related declines in muscle mass, strength and function, contributes to various negative health outcomes: disability, frailty, and hospitalization. Nutritional intervention is an effective strategy to counter sarcopenia and thereby prevent mobility disability. This study evaluated the efficacy of daily consumption of yogurt fortified with HMB, vitamin D, and vitamin C on muscle mass, strength, and physical performance independent of exercise in sarcopenic elderly. Methods: A total 66 sarcopenic elderly that were diagnosed by the Asian Working Group for Sarcopenia (AWGS) guideline, participated in 12-wk randomized, double-blind, placebo-controlled trial. Participants received either yogurt fortified with 3g HMB, 1000IU vitamin D3, and 500mg vitamin C in the intervention group(n=33) or plain yogurt in placebo group(n=33) for 12 weeks. We assessed body composition with dual-energy X-ray absorptiometry (DXA), muscle strength with a hydraulic handgrip dynamometer, and physical performance with 4-m gait speed in all patients. Nutritional status and physical activity were evaluated using mini-nutritional assessment (MNA) questionnaire and international physical activity questionnaire (IPAQ) before and after the 12-wk of intervention. Results: Compared with placebo group, yogurt fortification significantly improved handgrip strength (P=0.005) and gait speed (P=0.002) and decreased android distribution of fat (P=0.045). However in the intervention group, improvement in appendicular muscle mass was not statistically significant (P=0.06).Conclusion: Consumption of yogurt fortified with HMB, vitamin D, and vitamin C might enhance muscle strength and physical performance among sarcopenic older adults. This study reveals that specific nutritional intervention alone might be beneficial especially for those who are unable to exercise.
