Drug use evaluation of carbapenems and comparison with reliable guideline in children’s medical center hospital

Introduction: The inappropriate use of antibiotics remains the primary factor in antimicrobial drug resistance. In this study, we evaluated the use of carbapenems in the Children‟s Medical Center hospital in Tehran, Iran, and compared with reliable guidelines.Method: This cross-sectional observational retrospective study was conducted to assess rational use of carbapenems. The medical records of 250 admitted patients, who received carbapenems, were investigated during March to September 2017.Results: Of the 250 patients, 58.4% were male and 41.6% were female. Carbapenem administration in these patients was associated with general surgery and for the treatment of peritonitis (perforated and non-perforated appendicitis, cholecystitis,, rectal prolapse) (27.2%), cardiac surgery prophylaxis (21.2%), Pneumonia (18.4%), fever and neutropenia (10.4%), sepsis (10.4%), other cases such as meningitis, osteomyelitis, urinary tract infection and immune deficiency (12.4%). 76.8% of the prescribed antibiotics were indicated. Overall, about 41.6% of the total Carbapenem prescriptions and administration did not completely correspond to the sources (23.2% lacked indication, 22% incorrect interval and 4% Dose, duration of treatment was wrong). Among the incorrect indications, the most commonly prescribed cardiac surgery prophylaxis and non-perforated appendicitis. The highest total incongruity in prescribing was found in general surgery and cardiac surgery. 22% of the patients died. 78% of the patients had microbial culture, of which 38.5% were positiveConclusion: The results showed that nearly half of the patterns of antibiotic use in the hospital were inappropriate.
