6-month mortality rate in patients with heart failure

Background and Objective: heart failure is a complex clinical syndrome resulting from structural and functional impairment of ventricular filling or ejection of blood. Systolic heart failure patients are at risk of worsening and death. there are several stablished parameters used as routine tests all over the world, but the best prognostic tests are unknown in Iran society.Material and Methods: 100 systolic heart failure patients administered in autumn season of 2016 in Birjand Valiasr hospital were fallowed up for six months and adverse event including cardiovascular death or HF rehospitalization were recorded. 13 patients fallow-up data were excluded from study. laboratory tests were obtained at the first day of administration. All data were analyzed by SPSS V.24.Finding: we recorded 18 patients (20.7%) with bad outcomes and 69 patients (79.3 %) without outcomes had the mean age of 67.6(22.3) and 69(16.2) respectively. No significant relationship between sex(p=0.55), literacy level (p=.321), and NYHA classification (p=0.970) and mortality and morbidity were found. Hemoglobin level significantly predicts cardiovascular events (p=0.03). mean Hb levels 11.6 (4.3) vs 8.0(6.1) mg/dL between with and without outcome patients respectively. Urea(p=0.012) and acid uric (p=0.012) serum levels were significantly different between two groups. Na (<0.001) and K (<0.001) show significant different too. CRP, FBS, Creatine, cholesterol, TG, AST, ALT, HDL, LDL didn t show significant different in this study.Conclusion: elevated Hb could be considered a risk factor for whom they are at risk of death. Higher levels of BUN and acid uric which are related to kidney dysfunction are another risk factor that forecast shortness of life in HF patient. Na and K serum levels are considered as prognostic factors too.
