چکیده مقاله:

Background: Prostate cancer is a leading cause of cancer-related morbidity in men worldwide. In Iran, its incidence has increased over recent decades, yet provincial-level epidemiologic patterns remain poorly characterized. This study examined temporal trends, age-specific patterns, and spatial distribution of prostate cancer in Lorestan Province, Iran, from ۲۰۰۹ to ۲۰۱۹.Methods:Data were extracted from the Iranian National Cancer Registry for all histologically confirmed prostate cancer cases in Lorestan during ۲۰۰۹–۲۰۱۹. Age-standardized incidence rates (ASIRs) were calculated per ۱۰۰,۰۰۰ population using the World Health Organization (WHO) standard population. Temporal trends were assessed using joinpoint regression analysis, and spatial mapping was conducted to identify high-incidence clusters across counties. Diagnostic characteristics, including tumor grade and method of confirmation, were also analyzed. Results:The ASIR increased from less than ۰.۸ in ۲۰۰۹ to ۱۰.۸ per ۱۰۰,۰۰۰ in ۲۰۱۹, representing a ۱۳.۷-fold rise. Incidence was strongly associated with older age, peaking at ۱۴۶.۵ per ۱۰۰,۰۰۰ among men aged ۷۵–۷۹ years. Spatial analysis identified Dorud, Chegni, and Delfan as high-incidence areas. Among all cases, ۴۱.۷% presented with poorly differentiated (Grade ۳) tumors, and ۸۳% were diagnosed through histological verification. These findings suggest both improved diagnostic practices and persistent delays in early detection. Conclusions:Prostate cancer incidence in Lorestan has risen substantially, reflecting demographic aging, enhanced diagnostic coverage, and possible environmental or lifestyle factors. Targeted screening, improved diagnostic pathways, and focused etiological investigations are critical to mitigate the growing prostate cancer burden in Iran.
