Efficacy of three-dimensional sonography for diagnosis of uterine cavity abnormalities causing infertility: A cross-sectional study among the Iranian infertile population

Purpose: To evaluate the efficacy of three-dimensional transvaginal sonography (۳D-TVS) in diagnosis of congenital and acquired uterine cavity abnormalities among infertile women. Method & Materials: This was a cross-sectional study at Royan Institute, Tehran, Iran, from April ۲۰۱۲ and March ۲۰۱۳. All infertile women who were booked for hysteroscopy during this period of time were recruited in the study. Patients underwent preoperative ۳D-TVS by an expert radiologist with more than ۱۸ years experience in infertility workup. All hysteroscopies were performed by an expert gynecology surgeon with ۱۲ years experience. Patient’s history, ۳D-TVS findings and hysteroscopy results were assessed to collect data. Agreement between sonography findings & hysteroscopy results, sensitivity, specificity, positive & negative predictive values, and efficacy of ۳D-TVS in diagnosis of congenital and acquired uterine disorders were calculated by SPSS۱۸ software using Cross-tabs, Kappa test, and other statistics analysis. Results: Totally, ۱۵۳ women were recruited in the investigation. Patients aged ۳۰.۷۸±۴.۷۹ years in average and the duration of infertility was ۴.۲±۲.۹۱ years among them. Uterine abnormalities were grouped as “congenital” and “acquired”. Overall efficacy of ۳D TVS in diagnosis of uterine disorders was calculated ۹۲.۸% with Kappa value of ۸۲.۳, indicating a good agreement between ultrasound findings and hysteroscopy results. sensitivity, specificity, positive & negative predictive values of ۳D-TVS were calculated ۹۴.۵۵%, ۸۸.۳۷%, ۹۵.۴۱% and ۸۶.۳۶% respectively.
