Oil refineries represent highly complex industrial systems that process flammable, toxic, and pressurized hydrocarbons, exposing them to significant safety risks such as fires, explosions, chemical releases, and catastrophic failures. Effective risk assessment and management are essential to identify hazards, evaluate potential consequences, and implement controls that prevent major accidents, protect workers, safeguard communities, and ensure environmental sustainability. This article presents a comprehensive analysis of various risk assessment techniques applied in oil refineries, examining their methodologies, applications, strengths, limitations, and overall effectiveness in mitigating accidents. Key techniques reviewed include qualitative methods such as Hazard and Operability Study (HAZOP) and Failure Mode and Effects Analysis (FMEA), semi-quantitative approaches like Layer of Protection Analysis (LOPA) and Bowtie analysis, and quantitative methods such as Quantitative Risk Assessment (QRA). These tools are evaluated in the context of common refinery hazards, including process deviations, equipment failures, human errors, and external events. The analysis draws on recent literature and real-world incident case studies to assess how these techniques contribute to process safety management, hierarchy of controls implementation, and risk reduction. Findings reveal that no single technique is universally superior; instead, integrated approaches combining HAZOP for hazard identification, LOPA for barrier evaluation, and QRA for probabilistic modeling often provide the most robust risk insights. Challenges such as data uncertainty, dynamic operational changes, and human factors integration persist, while emerging digital technologies and risk-based decision-making show promise for future improvements. This review highlights the critical role of systematic risk management in reducing accident frequency and severity in refineries, offering practical recommendations for industry practitioners to enhance safety performance and regulatory compliance.
