Editorial The ongoing Iran–USA conflict presents a looming threat not only to immediate public safety but also to the long-term resilience of Iran’s healthcare system. While war-related casualties and infrastructural damage rightly dominate public discourse, an often-untold consequence facing Iran is the systematic disruption of medical education, portending delayed public health impacts that may unfold over decades. We see this upheaval not only in the education of current physicians, nurses, and allied health professionals now, but potentially in future generations to come.۱Iran’s healthcare system has historically faced pressures from economic sanctions, political instability, and prior international crises, demonstrating vulnerabilities in workforce sustainability and healthcare delivery.۲ Medical education in particular is impeded through restricted access to resources and research collaborations as well as global knowledge exchange.۳ Persistent conflict exacerbates these challenges by interrupting the continuity of training programs, limiting clinical exposure, and reducing opportunities and student desire for advanced medical education and research.۴Lessons from conflict-affected regions suggest that interruptions in medical education translate into long-term deficits in workforce capacity, clinical expertise, and system-level preparedness.۵ In Iran, where the healthcare workforce is already under strain due to demographic shifts, economic pressures, and prior sanctions,۶ a compounding effect of the current crisis may culminate in a generation of healthcare professionals with gaps in training and clinical competence or worse, that succumb to drop out or migration away from the conflict.۴ The collective impact to the healthcare workforce may very well lead to delayed recognition of disease outbreaks, suboptimal management of chronic conditions, and reduced capacity to respond to future emergencies, representing a protracted public health catastrophe.Proactive strategies are urgent and necessary to mitigate these consequences. These include remote and adaptive educational modalities, international academic collaborations, access to financial aid and safe shelter, and frameworks that promote institutional resilience in preserving core medical training during periods of conflict.۴ Without immediate intervention, the indirect toll of this conflict on Iran’s healthcare system may go well beyond immediate casualties and silently undermine public health for years to come.The inattention surrounding this issue is most concerning and when the ۲۰۲۶ Iran-USA war is remembered, human loss will be assessed by battlefield casualties. Yet Iran’s hospitals and healthcare system may exact a different toll; one of human capital erosion. Many preventable deaths may follow; the distant echo of a disrupted medical education and training system that the international community today must not ignore.
