High-Intensity Interval Training (HIIT) has gained significant attention for its efficiency and effectiveness in improving health and performance. This review synthesizes findings from ۲۰ studies to evaluate the physiological effects of HIIT across diverse populations, including athletes, patients with chronic diseases, and healthy individuals. The primary objective was to assess how HIIT influences cardiovascular, metabolic, and inflammatory markers, as well as exercise performance, and to explore its potential as a therapeutic and performance-enhancing intervention. A comprehensive literature search was conducted using databases such as PubMed, Scopus, and Web of Science, with inclusion criteria focusing on peer-reviewed studies published between ۲۰۱۱ and ۲۰۲۵. Studies were selected based on their relevance to HIIT and its effects on physiological outcomes. The analysis revealed that HIIT significantly improves cardiovascular function, including left ventricular performance and blood pressure regulation, particularly in individuals with heart disease. Metabolic benefits were evident in enhanced insulin sensitivity, reduced blood glucose levels, and improved lipid profiles, especially in patients with type ۲ diabetes and obesity. HIIT also modulates inflammatory and oxidative stress markers, with reductions in pro-inflammatory cytokines and increases in antioxidant enzymes. In athletic populations, HIIT enhances aerobic and anaerobic capacity, muscle mitochondrial biogenesis, and overall physical performance. Notably, HIIT has shown promise in managing conditions such as dysmenorrhea and non-alcoholic fatty liver disease, with positive impacts on hormonal balance and liver function. The research also highlights molecular adaptations, including changes in gene expression related to metabolism and inflammation. While HIIT is generally safe and effective, optimal protocols remain to be standardized. The findings underscore the versatility of HIIT as a powerful tool for improving health and performance, supporting its integration into clinical and fitness settings. Future research should focus on long-term outcomes, individualized protocols, and the synergistic effects of nutritional supplementation to maximize benefits.
