Chronic illness in children imposes multidimensional challenges on families and healthcare systems. Combining self care education and family centered care (FCC) principles with telenursing has shown promise for enhancing clinical outcomes and family empowerment. This systematic review explores the triadic relationship between these concepts in pediatric chronic disease management. Methods: Following the PRISMA ۲۰۲۰ guideline, databases including PubMed, Scopus, Web of Science, CINAHL, SID, and Google Scholar were searched from ۲۰۱۵–۲۰۲۴ using keywords “telenursing,” “family centered care,” “self care education,” “pediatric chronic disease,” and “digital health.” Out of ۳۲۶ retrieved articles, ۲۲ met inclusion criteria (RCT, quasi experimental, or cross sectional designs focusing on children). Results: Structured self care education delivered through telenursing platforms improved adherence to therapy, emotional adaptation, and symptom monitoring. Family centered interventions enhanced communication, shared decision making, and satisfaction. Joint implementation of telenursing and FCC created a synergistic effect, increasing parental self efficacy and decreasing re hospitalization by ۲۵–۴۰%. Programs combining asynchronous education with synchronous teleconsultation were most effective. Conclusion: Integrating self care education, FCC, and telenursing represents an innovative care model that significantly strengthens continuity of care and empowers families of chronically ill children. Future research should address cultural adaptation and cost effectiveness in diverse healthcare settings.
