The incorporation of artificial intelligence (AI) into surgical instruments, specifically intelligent scalpels, marks a groundbreaking development in the field of medical treatment. In this narrative review, we explore the advancements associated with AI-operated surgical technologies, with a focus on enhanced precision, support for the operating room team in the decision-making process, and improved patient outcomes. In a time span of ۲۰۱۰–۲۰۲۵, an analysis based on the collected data of peer-reviewed studies, clinical trials, and regulatory frameworks unfolds the massive transition that the intelligent scalpels could potentially bring about, as it discusses the hurdles all the way from ethical to technical to practical ones. The results of the research pinpoint the importance of machine learning, real-time analytics, and robotics in not only error elimination but also the optimization of workflows and including all people, regardless of their position in society, in high-quality surgical care.
