COVID-۱۹ in Intensive Care Unit Admitted Patients: A cross-sectional study on the related factors

The ongoing COVID-۱۹ pandemic has posed significant challenges to healthcare systems worldwide, particularly in intensive care units (ICUs), where critically ill patients are treated. This cross-sectional study aimed to investigate the demographic characteristics, comorbidities, and mortality factors associated with COVID-۱۹ patients admitted to the ICU of Imam Reza Hospital in Mashhad, Iran, to optimize patient management and improve outcomes. A total of ۴۰۹ ICU patients were included in the study with a confirmed COVID-۱۹ diagnosis based on RT-PCR tests, ۶۱.۵% of whom were males, with a mean age of ۵۶.۳۲ ± ۱۵.۹۲ years. The most common presenting symptom was dyspnea (۹۵.۶%), followed by weakness (۸۲.۷%), chest discomfort (۷۷.۸%), headache (۶۶.۱%), fever (۶۵.۴%), and cough (۴۷.۸%). Hypertension (۵۲.۴%) and diabetes (۵۰.۲%) were the most prevalent comorbidities among the participants. The overall mortality rate was ۵۵.۹%. Factors significantly associated with higher mortality included older age (p<۰.۰۰۱), confusion at admission (p<۰.۰۰۱), diabetes (p=۰.۰۰۶), hypertension (p<۰.۰۰۱), ischemic heart disease (p=۰.۰۱۴), higher heart rate (p<۰.۰۰۱), respiratory rate (p<۰.۰۰۱), and lower SPO۲ (p<۰.۰۰۱). Deceased patients also had significantly higher blood glucose (p<۰.۰۰۱), urea (p<۰.۰۰۱), creatinine (p<۰.۰۰۱), CRP (p=۰.۰۱۷), INR (p=۰.۰۲۶), and CT scores (p<۰.۰۰۱) compared to survivors. The findings underscored the importance of identifying high-risk COVID-۱۹ patients at the time of ICU admission. These results suggested that patients exhibiting these characteristics require close monitoring and early, aggressive interventions from the onset of hospitalization to improve their clinical status and reduce the risk of mortality.
