The increasing understanding of the various types and roles of non-coding RNAs has paved the way for their application as innovative therapeutic strategies in managing chronic conditions like hypertension. Although most preclinical and clinical research on hypertension centers on prognosis and treatment, there remains a lack of targeted therapies for primary and essential hypertension. Limited treatment effectiveness and poor long-term adherence highlight the urgent need for novel, mechanism-driven approaches to address the disease's molecular foundations. Synthetic RNA molecules known as antimiRs and naturally occurring circular RNAs (circRNAs), which act as microRNA sponges, show promise in regulating the renin-angiotensin system (RAS). These non-coding RNAs offer substantial potential as groundbreaking strategies for treating primary and essential hypertension. This review seeks to thoroughly examine the interactions between circRNAs and miRNAs in the context of hypertension through their influence on RAS modulation, detailing their mechanisms of action, therapeutic benefits, and significant translational barriers, such as delivery challenges, unintended effects, and safety issues. It also discusses the need to enhance delivery methods, confirm long-term treatment efficacy, and navigate regulatory processes to advance these ncRNA-based therapies into preclinical and clinical stages. Additionally, the review emphasizes the promise of these RNA-based targets to address current therapeutic gaps and support the development of precision medicine in treating hypertension and associated cardiovascular disorders.
